Introduction to Goals, Conditions, and Treatment Plans

Keeping track of the client's medical conditions and goals goes hand in hand with standard compliance and upholding the organisation's service standards.

It's important to have a conversation with your clients about their goals, what they would like to achieve, and how they would like to achieve their goals. When delivering services for the clients, staff members need to check if the client has any pre-existing conditions that might block staff members from delivering services.

There is a separate section of the client profile dedicated to the client's goals, conditions, and treatment plans are maintained. When you add services to a client plan, or as one off services they can be linked back to the goals and condition treatment plans that the service is supporting, ensuring all services are delivered to improve the care outcomes for the client and making it easy to trace and review with auditors. 

When to Add or Update Goals, Conditions, and Treatment Plans

We encourage collecting these early during the discussion with the client and adding them even if in name only so they can be linked to the services that are planned to support them. They are often updated as part of the onboarding once a client has signed their contract, and it's also essential to update this section when:
  1. The client has new goals
  2. based on the client's diagnosed medical condition changing
  3. New prescribed medication or activity treatments
  4. Associating goals and conditions to services
  5. During reviews and assessments

Adding a New Goal and Condition on the Client's Profile

Each organisation will have its own unique approach to the detail that should be captured with respect to goals, conditions and treatment plans, and we encourage you to set a standard approach to how these are documented within the framework provided by CareVision. This will help your teams to ensure consistency of delivery and ease of interpretation of the information on the client profile. For example, some clients will insist that the Detail or Treatment Plan have a prescribed set of headings and each heading addressed. Once you have agreed to a standard for use, you can follow the instructions below to create Goals and Condition Treatment Plans for your clients. 
  1. On the Client's Profile, find the GOALS, CONDITIONS & TREATMENT PLANS Section. Click to expand.



  2.  Click + Create New Condition and Treatment Plan
  3.  Fill out the Create Condition And Treatment Plan Popup.


  4. Click on the Patient Condition Type field. You can tick one or more from the pre-added list.
    PRO TIP: You can also add a new Condition to the list by typing the condition name on the field and clicking on ADD CONDITION.



  5. Provide a Treatment/Action Plan Description. This section is important; it should have as much detail, background to the item as possible, along with justification for the planned activities. Consider including here links to evidence-based justifications for treatments or planned activity—the history of the condition or where the goal originated from. This section is a rich text editor and will allow you to format the Plan Description. 

  6. Provide a Goal Description. It can be a one-liner describing the main aim/goal. 
    i.e. client wants to improve confidence walking up and down the stairs and strength for moving around

  7. Trackables
    Staff Members can capture, record and track the progress or status of each goal as clients work on reaching their desired health goals. 

    1. To add a Trackable, click on the ADD TRACKABLE button. It can be their Blood Pressure, Mood, Weight, or Memory.

    2. Add a Target Date or a deadline for tracking the health goal type

    3. Add a Minimum Target Value or a Maximum Target Value for the trackable. 

    4. Tick the must be maintained checkbox if the trackable must be kept within range of the target values.

    5. Click SAVE to keep the Trackable.

  8.  You can also choose to set or add a Reminder. This will send an notification to the Client and the attendees

  9. Add TREATMENTS.
    There are two types of treatments.

    1. Medication Treatments - Prescriptions, medical advice provided by doctors

    2. Activity Treatments - Advised physical activity, exercise or therapy
      (See Adding Treatment Plans section)


  10. Upload Attachments. Add images or documentation such as photos of prescriptions or medical certificates. 

    1. Image files - JPG, PNG, GIF

    2. Video Files - MP4, MOV, AVI, WMV

    3. Documents - doc, docx, PDF

  11. When you're done adding all the details, click SAVE or you can also tick on "Create Another" if you have another goal to add.


Adding Medication Treatments



To keep a record of current medications, click on ADD TREATMENT under the Medication Treatments section fill out the following details:

Refer to the Doctor's prescription for the following details.
  1. Name of Medication 
  2. Prescribed by - The Prescribing Doctor's name
  3. Dosage of Medication  
  4. Quantity per Dosage 
Other details:
  1. Self Administered - If the client needs extra help to take the medication, leave the checkbox unticked.
  2. Instructions for Taking Medicine - Add steps on how to take the medicine. You can also add warning notes.
  3. Medication Schedule
    1. Allocated to - If the medication needs to be administered by a member of the staff, select the staff name on the dropdown.
    2. Frequency - If the medication needs to be taken Daily, Weekly, Monthly (Date Based), Poly-weekly, Fornightly or Once-off
    3. On (Date) - Date when the client starts taking the medicine
    4. Start Time - Time a staff member needs to be in the client's side when taking the medicine
    5. Duration - Number of minutes it will take for the client to take the medication.
  4. Attach the prescription on the Attachments section.
  5. When you're done adding the Treatment Plan, don't forget to click SAVE

Adding Activity Treatments


To keep a record of Activity Treatments such as exercise or therapy, coordinators need to fill out the following fields:

Refer to the Doctor's prescription for the following details.
  1. Activity Name
  2. Prescribed by - The Prescribing Doctor's name if it's medical-related. For exercise or physical training, a gym instructor or a Physical Therapist will be the prescriber.
  3. Instruction for Activity - Any special instructions like a specific exercise regimen or meal plan for dieting.
Other details:
  1. Activity Schedule
    1. Allocated to - If the activity needs to be facilitated by a member of the staff, select the staff name on the dropdown.
    2. Frequency - If the medication needs to be taken Daily, Weekly, Monthly (Date Based), Poly-weekly, Fornightly or Once-off
    3. On (Date) - Date when the client starts taking the medicine
    4. Start Time - Time a staff member needs to be in the client's side when taking the medicine
    5. Duration - Number of minutes it will take for the client to take the medication.
  2. Attach the prescription or instructions on the Attachments section.
  3. When you're done adding the Treatment Plan, don't forget to click SAVE
