Setting Up Trackables

Overview

Setting up Trackables is a method used to monitor a client's progress in achieving their goals over time. During every visit, these values are recorded and updated, providing a clear and consistent view of the client's progress, or by manually adding the values via Web Portal (Goals section). By regularly tracking these metrics, Care Coordinators can evaluate the effectiveness of the care plan and make necessary adjustments to ensure that the client remains on track toward meeting their objectives.

Steps

Setting the Trackable Form

Settings >> Form Format

Look for the Form to be used to submit value to your Trackables


At the very bottom of the Form setup, make sure that the Form Property is tagged as "Trackable".



Save the Form.

Associating the Trackable Form to the Care Domain

Settings >> Care Domains

Open the applicable Care Domain created and at the very bottom of the set up, Linked Form Formats, attach the Form configured on the earlier step.


Save the Care Domain

Setting up the Goals, Treatment Plans and Assessments

Client Profile >> Goals, Treatment Plans and Assessments

Care Domain dropdown - select the Care Domain that you have your form trackable linked on.



Tick the checkbox to enable the linked form. This is where you can manually add 'Latest Reading' by clicking the pencil icon on the bottom-right.


Go to the very bottom of the Goals section to Save the Goal

Adding Trackable

Client Profile >> Goals, Treatment Plans and Assessments

Click on the Goal created



Click ADD TRACKABLE





  1. Health Goal Type - select the applicable.
  2. Target Date
  3. Minimum Target Value
  4. Maximum Target Value
  5. Latest Reading
Save the Trackable

Checking the Trend

After adding a Trackable, it will display a new section for us to be able to see the trend of the values captured by the form.



Adding Values

Via the linked Form

Click the Pencil Icon and put in a new Value, then save by clicking the Check icon


We need to resave the whole Goal so the Latest Value will appear.

Make sure the the Next Review By and Review Type fields are populated when saving the whole Goal.

Reopen the Goal, and we should now see the Latest Reading



Chart

This is where you can visually check the trend of the values captured over time.

Click on the Chart icon





Submitting Values via Care Plan Bookings

  1. Make sure that the Client Care Plan has the applicable Care Domain Selected
  2. The Form Format is also selected on the Client Care Plan
This way, all bookings that have the Form will have to be filled out by the staff, and those values will also appear on the Chart.
