How to Create Forms for Clock In
Clock-in Questionnaires
The start of a shift can be the best time for staff members to manage administrative tasks before going about their day. Some organisations require staff to complete a form before clocking-in for a number of reasons including:
- Health and Safety Checks
- Confirm COVID status and close contact status
- Compliance Checks
- Surveys
- Equipment and vehicle information
Terms
- Form Format - The Form Template that was added to the Settings area as part of the system configuration. This is accessed by admin users. Examples are Health & Safety Check, COVID questionnaire, vehicle inspection report. To learn how to build a form format check out the Building Form Formats article.
- Form Submissions - The form created or answered by any staff member. Form submissions are created after selecting a form format to use.
- Clock on questionnaire - A form that will be displayed after a staff member clocks in for their shift on the mobile app.
Setting up Clock-in Questionnaires
The following steps are for users that are currently signed in on the CareVision web portal as a coordinator.
STEPS
- On the Navigation Menus, go to Settings and scroll down to Form formats.
This page will show a list of your active forms formats.
- Click on the Create a new form format Button.

- Complete all of the sections in the CREATE FORM FORMAT pop-up as normal
- In the Special Purpose Form section question, click the dropdown and Select ‘Clock On Questionnaire”
- Once you have added all of the questions to your form don't forget to click SAVE.

- This form will then appear as a questionnaire for all allocated staff when they are clocking in.
Accessing Clock-On Questionnaires
On the mobile app, when a staff member clocks in for their shift a pop-up confirming the start time will appear.
If the organization does not have any Clock On Questionnaires created the clock-in is finished and the staff member begins their work as normal.
If the organization does have a form format set as a Clock On Questionnaire the form will pop up.
The staff member should complete the form and submit it. This will make it available for portal staff to access through the CareVision web portal.
