Forms for Specific Service Types Explained


For some types of service there may be a need to collect additional standard information about the service.
An example might be a medication service requiring staff to complete a medication checklist recording which medications were administered, in what format and and what time.
This can be accomplished by associating form formats with a service, allowing it to be utilized anytime a service booking is made.

Adding a standard Form Format to an Organisation Service

  1. On the sidebar of your organization portal, click on SETTINGS, SERVICES to access the service templates for your organisation.


  2. Choose the service you would like to add a form to from the list of services. - e.g. Personal Care & Home Support


  3. A popup will display containing the details of the service you chose.
    Under the SERVICE DELIVERY DETAILS section of the popup, you can now add a form to that service by selecting a form from the dropdown list of the Form Format field.


  4. Once you are done with the change, don't forget to click SAVE on the lower right corner of the popup.


Now that you have associated a form to one of your services, it will now become available to future service bookings using that service. However, published service bookings prior to our update using that service will not be affected.


Filling out the form on the mobile application

  1. Login on the CareVision+ mobile application on your device and go to your scheduled service booking by clicking on the view details link at the bottom of the booking card.
  2. Once you are on the service booking details, there is a tab called Forms  that you can click to view all the forms associated to that service booking.
  3. Select the form from the list you need to fill out by clicking on its name.
  4. The form fields will now display for you to fill out, don't forget to click on the button at the lowest part of the form to save your changes.

Add Specific Form to an Individual Client Service Booking or Care Plan 

While the above steps show how you can add a standardised form for a service at the organisation level, this can be customized at the client level.
