Client Goals Condition and Treatment Plans

The coordinators can manage the Goals, Conditions, and Treatment Plans for a client. It will help carers to know the current conditions or goals of the client specially if they are assigned to assist the client based on their need and to achieve the best health care service for the client.


Accessing the Client Goal Condition and Treatment Plans


Here are the steps on how to access the Client Goals, Conditions and Treatment Plans

1. In your CareVision Management Portal, go to the Menu Tab and click PEOPLE and then go to CLIENTS




2. In the list of Active Clients, you need to choose a client by clicking their name. In this image, we used Cate Jones as an example.




3. When you are in the Client's Profile, please go to the Goals, Conditions & Treatment Plans.

The Creation of the Client Goals, Conditions and Treatment Plans



After you expand the GOALS, CONDITIONS & TREATMENT PLANS section, you will see a button that has a label of + CREATE NEW CONDITION AND TREATMENT PLAN. You need to click it to create the goals, conditions, and treatment plan for the client.




Below is the modal form for creating the client's goal, condition, and treatment plan where you need to fill in the required fields. 





The Patient Condition Type Field

This is the first required field and you have to choose from the available options.
You can choose the value by opting for one or more available patient condition types. If you wish to use all condition types, click the SELECT ALL button.




If in case you cannot find the necessary patient condition type, you can add it by following these steps:

1. Click the drop-down section of the Patient Condition Type and click the ADD CONDITION button.




2. In the search field, please provide the specific patient condition type that is not available in the selection. After that, you need to click the ADD CONDITION button to add the new Patient Condition Type. Please note that the newly added Patient Note Type will be added to the list of the Organisation's Patient Condition Types after the saving of the entire creation of the client's goal, condition, and treatment plan.




The Treatment / Action Plan Description Field

The Treatment / Action Plan Description is an optional field where you can add the details that relates to the creation of the Client's Goal, Condition and Treatment Plan.


The Goal Description Field

The Goal Description is a required field. You need to provide the health goal description of the Client's Goal, Condition and Treatment Plan.


The Trackable Field

The trackable is an optional field, but you can choose to add if needed. Here, the carers can record and track the status or progress of the client as they work on reaching the health goals. It can be the client's Blood Pressure, Mood, Weight or Memory




The Reminder Field

This is an optional field. You can choose to set or add a Reminder and this can be related to the client's current health goal.







The Date Reported Field

This is a required field where the carer needs to provide the date when the client's goal, condition and treatment is created. 


The Date Resolved Field

This is an optional field where the carer can provide a date if the client reached the health goal.


The Treatment Section

This section is comprises of two sub-section:

1. The Medication Treatments section contains the information on the medicines that were prescribed to the client as well as the instructions for taking medicine. In this section, you can also set the schedule for the medication and allocate it to the staff. 





2. The Activity Treatments section contains the set of information related to the required activity for the client. You can also set a schedule for it and allocate to a carer.




The Attachment(s) Section

We allow carers to attach related information for client goals, conditions, and treatment plans. Here, both image and non-image files are allowed for the attachments.




Once you've provided the necessary information, click the Save Button. 

The new Health Goal or Condition should be added the the list section of the Client's Goal, Condition and Treatment Plan and it is editable,
