Minor improvements and fixes

1. Update Xero format for invoice export and handling of GST naming 
   
2. Update the Care Plan Cost Estimates to be based off the new and improved Quote Calculation API for improved accuracy
   
3. Allow scheduling area to support showing of current service name of a published care plan items where the service on the published item has been changed 
   
4. Form submissions action board filter updates to ensure more meaningful and useful filtering
